Founder and CEO Baheejah Mahdi, MSN, MSM, RN, BS is a Philadelphia native with more than 25 years of experience in the health care profession. Baheejah started her health care employment journey working as an billing clerk and emergency room registrar. Later, Ms. Mahdi began working as a family social worker and volunteering with several non-profit community organizations. Through many volunteer opportunities, she has learned that underserved communities are not just a case of low income individuals, but also a lack of access to health care, schools, social security and many other social determinants of health. Although these patients need immediate attention medically, she believes that a lack of social support in the community is equally as deterring for their future health.

The underserved has motivated Ms. Mahdi to strive for a career in nursing and focus on developing, planning, implementing and evaluating programs within those communities. She continued her education and journey through a registered nursing program and has been very influential in the profession of nursing. Combining her social work and nursing experience, Ms. Mahdi considers herself a nurturer, someone who finds happiness in helping others especially through her passion and dedication for both public health nursing and volunteerism.

Ms. Mahdi worked in a primary care health setting in the role of a registered nurse navigator and as a home care nurse she encompasses strong experience in care coordination, social work and delivering quality care to patients. Working with patients from all walks of life and those with diverse medical and mental health conditions, Ms. Mahdi has been able to apply her professionalism and skills to the home care industry. Her role affords her the opportunity to work closely with the elderly and adults with physical and intellectual developmental disabilities in the community where she is totally committed to address the social determinants of health that individuals face by providing hands-on assistance and ensuring continuity in care, closing care gaps, improving access to reliable community resources and working closely with interdisciplinary teams to help patients achieve their health care and personal goals as well as to improve health and social outcomes.
